He served as Director of a Community Mental Health Centre (1995–2019), Head of the Department of Mental Health in Trieste (2012–2019), and Director of the WHO Collaborating Centre for Research and Training (2009–2019). His work focuses on deinstitutionalization, crisis care, recovery-oriented services, and human rights in psychiatry. A co-founder and Chair of the International Mental Health Collaborating Network, he is Regional Vice President for Europe of the World Federation for Mental Health. This model was subsequently adopted across England as the national framework for community mental health services. He has held senior leadership and academic positions, including Medical Director of the North Birmingham Mental Health NHS Trust and Professor of Community Psychiatry at the University of Birmingham. He has also held honorary appointments at the University of Warwick and the University of Glasgow. Internationally, he has contributed to mental health policy and service development in several low- and middle-income countries through consultancy work with the World Health Organization and the Asian Development Bank. More recently, he led the development and implementation of the Ethnicity and Mental Health Improvement Programme in South West London, aimed at addressing disparities in mental health outcomes and service access.
